What To Expect On Your First Visit:

Insurance And Setup:

When you schedule your initial visit we will collect your insurance info.  Try and and have your insurance card on you when you make your appointment.  If you cannot find it, we will need your member ID # (subscriber ID), date of birth,  insurance company name, and their phone number if available.  We will ask you to arrive 15 minutes before your initial appointment to fill out new patient paperwork.

Confirming Your Appointment:

We will text you and call you prior to your appointment before your first visit.  Please reply to our text or call to confirm.  We ask that you provide us 48 hour notice of change or cancellation to your appointment

What we will do first:

Whether you are being seen for a toothache, a chipped tooth, or want a teeth cleaning; we will need to take X-Rays and do an exam first.  We cannot guarantee you a cleaning on your first visit.  If you are expecting that, please call the office to let us know that.  The doctor will review your x-rays and exam with you and let you know what is needed.

Financial Considerations For Your Visit:

We want to fix your toothaches, chipped teeth, and get your teeth cleaned all on your first visit if possible.  If you desire to get these done after the doctor diagnosis, please let us know.  If you want to obtain financing, payment plans, for your payments please follow the following instructions:

Patient Financing:

·         We use a 3rd party company called Carecredit.  They are owned by a very reputable company, Synchrony Bank, the 9th Largest Bank in America.

·         For Treatment that you need to pay $600 or less, we partner with them to offer payments of 6 Months with no interest.  For Payments between $600-$1200 we partner with them to offer payments of 12 Months with no interest.  For payments of $1200 and more we offer payments of 18 months with no interest.

To Set this up before your visit go to:

1)      Go to https://www.carecredit.com/

2)      Click Apply on the Upper Left Hand Corner

3)      When you get approved, either bring your account # to the office or we can look it up with your Social Security # and Zip Code

4)      Let us know that you want to use your CareCredit approval line of credit and we will provide you with 0% interest
